KHCA023 - Care Plans for SSD's

This session with Linda Farrar, RN, LNHA will cover the intricacies of care planning for social service designees in Long-Term Care Communities including the federal regulatory requirements related to creating the care plan. Linda will also dive into Base Care Plans, including when they are developed and what should be included. There will be time devoted specifically to answering questions from participants at the end of this 90-minute webinar.

Learning Objectives for this webinar are:

Participants will understand the regulatory requirements related to development and implementation of individualized, timely, comprehensive, person-centered care plans
Participants will understand the purpose of care plans related to psychosocial wellbeing for each resident
Participants will understand when a Baseline Care Plan must be developed and what should be included in a Baseline Care Plan
Participants will understand the key components of psychosocial care plans and how to write the required components of the care plan
